Title: Intake and Engagement Coordinator
Location: South Sound Treatment in Motion (TIM) Mobile unit
This is an non exempt full time position
Hours 5:00 AM – 1:30 PM, Monday through Friday with the occasional Saturday shift (Saturday clinic hours are 6:00 AM – 2:00 PM). Any changes to work schedule is negotiated with employee's supervisor.
The compensation range is $28.84-$30.16/hr.

Job Summary:
The Intake & Engagement Coordinator role is part of the Behavioral Health team that supports ETS’ mission through onboarding and engagement of individuals seeking services.
- This role makes a critical difference at ETS by This position is responsible for facilitating the intake process, ensuring a smooth transition into services, maintaining high levels of participation during the first 90 days of care, and engaging those identified needing continued stabilization. The role requires close collaboration with referral sources and internal teams to link individuals to the appropriate services, such as case management, counseling, medical services, and other support, and includes ongoing follow-up to ensure sustained participation.
- The position is full-time onsite, and the days and hours are M-F 5:30AM-2:00PM.
What you contribute to this role – Responsibilities:
- Help ETS succeed in carrying out our mission through working together with other staff to transform systems of harm and inequity to create different approaches to improving community health and safety through addressing substance use and homelessness.
- Assist with the organization-wide initiative to reimagine our interconnectedness within our community to overcome the aspects of our society and organizational culture shaped by white supremacy and settler colonialism.
Intake Process Coordination
- Participant Engagement & Welcome: Serve as the first point of contact for individuals entering the organization. Ensure they feel welcome and provide them with an overview of available services.
- Comprehensive Intake Assessments: Collect and document personal information, service history, and immediate needs. Perform initial screenings for eligibility and suitability for internal programs.
- Service Plan Development: Work with individuals to create initial care plans, connecting them to appropriate services based on their immediate needs and long-term goals.
- Consent & Documentation: Ensure all required consent forms and documentation are signed and filed according to organizational policy and regulatory requirements.
Service Linkage & Coordination
- Appointment Scheduling: Coordinate appointments for individuals with internal services, including case management, counseling, medical appointments, and other necessary resources.
- Referral & Follow-up: Assist internal service providers with making referrals to outside agencies and help participants navigate barriers to attending referral appointments.
- Service Navigation: Act as a liaison between individuals and service providers within the organization, ensuring timely access to services and addressing any logistical challenges e.g., transportation, scheduling conflicts.
- Barrier Identification: Identify potential obstacles to service engagement, such as financial constraints, childcare, or transportation, and work with internal teams to find solutions.
- Collaboration with Providers: Facilitate warm hand-offs to relevant team members (case managers, counselors, medical professionals) for more specialized services, ensuring smooth communication and a cohesive care approach.
Ongoing Engagement & Support
- Check-Ins & Monitoring: Regularly contact individuals to check their progress and adherence to their service plans, including attending scheduled appointments and engaging with provided resources.
- Follow-Up Calls: Conduct phone outreach to individuals who miss appointments or services, discussing barriers to attendance and helping to reschedule as needed.
- Outreach & Engagement: Maintain contact with participants through various methods (phone, email, text, in-person visits) to ensure consistent engagement.
- Engagement Reporting: Document participation levels, outreach efforts, and progress updates in the organization’s Electronic Health Record (EHR) and other tracking systems.
Crisis Management & Problem Resolution
- Crisis Response: Recognize and respond to crises, including mental health, substance use, or housing instability, by quickly linking individuals to appropriate services or support.
- Conflict Resolution: Assists participant and provider in resolving conflicts or misunderstandings while maintaining alignment with the agency's philosophy of care and participant safety agreement. Problem-Solving: Assist individuals in resolving any issues that may hinder their progress, whether related to service access, personal challenges, or external factors.
Data Collection & Reporting
- Accurate Record-Keeping: Maintain detailed and up-to-date records of all participant interactions, referrals, and follow-up efforts in accordance with organizational standards.
- Outcome Tracking: Monitor key performance indicators (KPIs) such as medication adherence, appointment attendance rates, engagement levels, and service completion rates, and report these metrics through established agency channels.
- Compliance & Confidentiality: Ensure all documentation is completed in compliance with applicable laws and internal policies, including HIPAA and 42 CFR Part 2.
Collaboration & Team Coordination
- Team Meetings & Case Conferencing: Participate in regular team meetings, case conferences, and staff development activities to stay informed about available services and participant progress.
- Feedback Loop: Provide ongoing feedback to other care team members regarding participant needs, potential service gaps, and opportunities for improvement.
- Service Integration: Work closely with partner agencies and external service providers to ensure coordinated care, making referrals as necessary for services not provided internally.
Community Outreach & Education
- Service Promotion: Assist in educating potential participants about the services offered by the organization through outreach events, presentations, and resource fairs.
- Community Resources: Maintain a thorough knowledge of external resources and services in the community, providing participants with referrals when internal services do not meet all their needs.

Requirements
What you bring to this role – Qualifications:
Education and/or Relevant and Lived Experience (if applicable):
- Associate in social work, human services, psychology, or related field preferred.
- 1-2 years of experience in case management, social services, intake coordination, or a similar role preferred.
- Familiarity with intake procedures and participant engagement strategies.
- Active, restriction-free Washington State Driver's License

Skills needed include basic functions such as utilizing MS Outlook email and calendaring programs (or equivalent) and sending attachments, using MS Teams or equivalent chat, call, and videoconference features, and navigating search engines such as Edge or Google and carrying out browser searches and website benchmarking steps.

Physical Conditions and Requirements:
- The employee may be exposed to illicit drug residues and fumes or other bio-hazardous materials when carrying out job functions. There is also potential for exposure to bloodborne pathogens. ETS will provide employees with appropriate training to limit the risk of exposure to bloodborne pathogens. Policies and procedures are in place addressing each item specifically.
- The employee is regularly required to sit; use hands to finger, handle or feel objects, tools, or controls; reach with hands and arms and talk or hear; frequently required to stand, walk, and kneel; occasionally to climb balance, or stoop; rarely to crouch or crawl.
- The employee must occasionally lift and/or move up to 30 pounds. Specific vision abilities required by this job include close, color, and peripheral vision and the ability to adjust focus. The noise level in the work environment is moderate.
