REQUIRED EDUCATION, TRAINING AND EXPERIENCE; CERTIFICATION/LICENSURE:
Graduate of an accredited school of pharmacy. Current license in good standing and registration with State of Washington Board of Pharmacy. Requires current Basic Life Support (BLS) certification and must obtain Advanced Cardiac Life Support (ACLS) certification within 30 days of hire date.
SPECIAL EQUIPMENT, SKILLS OR OTHER REQUIREMENTS:
Must be able to organize efficiently. Good interpersonal skills; good verbal and written communication skills. Must complete yearly Pharmacist specific competencies. Use of personal protective equipment as appropriate, documentation of immunization/immunity against hepatitis B, and training related to blood-borne pathogens is required.
WORK ENVIRONMENT AND HAZARDS:
Adheres to all State and Federal regulations pertaining to hazardous materials and BBP. May be exposed to labeled bio-hazard substances and infectious material. Must be able to lift up to 40lbs in IV solutions.
WORK CONTACT GROUP: All employees, physicians, patients, family and support members, vendors and visitors.
SPECIAL PHYSICAL DEMANDS:
Stands and walks for most of the working day. Must be able to maintain a rigorous schedule; some lifting, bending and carrying.

ESSENTIAL FUNCTIONS:
- Interprets medication orders (verbal and written) and transcribes or verifies to/in a computerized patient medication profile. Maintains accurate and complete medication profiles.
- Compounds and dispenses pharmaceuticals, including the use of aseptic and sterile techniques.
- Works with patient/other healthcare professionals in designing, implementing, and monitoring a cost effective therapeutic plan to produce specific patient outcomes.
- Identifies and resolves potential/actual drug related problems including drug interactions / incompatibilities, adverse drug reactions, allergies, use of drugs without indications; seeks out and reviews identified necessary laboratory data; documents all activities/actions taken.
- Sustains the formulary by minimizing nonformulary procurements, utilizing therapeutic substitution protocols and promoting rational drug therapy selection
- Reviews/interprets cultures and susceptibility data for antibiotic appropriateness and recommends changes, as needed.
- Accurately monitors drug therapy, including aminoglycosides, Coumadin, vancomycin or other drugs as requested, selects dosages based on patient's lab data, condition and other relevant data; takes action as needed.
- Works with patient/other health professionals to prevent potential drug related problems; documents interventional activities and their outcomes.
- Coordinates daily activities of pharmacy support personnel to promote efficient use of personnel.
- Ensures/controls proper storage/security of drugs throughout the hospital according to state/federal laws and regulations, and hospital policy.
- Consults with and provides information to physicians, nurses and other necessary hospital staff concerning new drugs, drug warnings, drug therapy, drug actions and other pharmaceutical information necessary for proper/effective pharmaceutical care; documents interactions.
- Conducts self in a professional manner maintaining an atmosphere of hospitality within/without the department, including interactions with other hospital personnel, patients, and visitors.
- Carries out policies and directives of Pharmacy and Therapeutics Committee, including medication utilization evaluations and encourages compliance with hospital formulary.
- Provides accurate, adequate and timely drug information to the professional staff.
- Participates in the development and presentation of orientation, education and training programs to the pharmacy, medical, nursing and other staffs.
- Participates in required and voluntary continuing education in an effort to reach personal and departmental goals.
- Effectively explains usage of products or devices to patients as requested by physician, nurses or patient, including but not limited to, Coumadin, insulin, and cardiac drugs.
- Uses an understanding of the principles of growth and development to assess each patient's age specific needs and provide age specific treatment and care.
- Understands and complies with all department policies, including handling of chemotherapy products, hazardous waste disposal, emergency procedures, fire safety, infection control, universal precautions, attendance and leave policies.
- Participates in department quality improvement plan, makes constructive suggestions for improvement of the plan, and documents own identification of problems and efforts toward resolutions.
- Resolves all problems that arise within the shift or passed on from previous shift; if resolution cannot be accomplished within the shift, efforts toward resolution are documented and communicated to a pharmacist on next shift; problems that cannot be resolved within 24-hours of initial occurrence are documented and referred to the pharmacy and/or nursing manager.
