Full-time Nurse Practitioner position available in Greenville, OH, US - Work schedule : Monday to Friday, 8 : 00 am to 4 : 30 pm - Weekend, holiday, and on-call duties required - Location : Mercer office with potential travel in service area - Duties may include coverage at inpatient care center - Support Medical Director at EverHeart Hospice
Responsibilities :
- Guide hospice and palliative care program
- Provide medical guidance to physician employees and hospice team
- Act as liaison between program, local physicians, and healthcare professionals Offer direct medical treatment to patients at homes, hospitals, or nursing homes - Expected to demonstrate clinical leadership and excellence

Routine Homecare :
Oversee the implementation of the plan of care for patients / families and in coordination with the patient’s physician, evaluate and change the plan of care as necessary to meet the needs of the patient / family.Consult with the Medical Director when the plan of care is outside of standardized practice and protocols and specialty consults are needed to manage patient care.Prescribe medications to the extent delegated and licensed after collaboration with the Medical Director.Update the interdisciplinary team on changes made to the patient’s Plan of Care.Collaborate with the Admissions Team and Team Managers concerning admissions and referrals.Providing services, clinical input, and oversight for all patient treatment plans is consistent with hospice and palliative care philosophy.Participate in all components of hospice as requested, including but not limited to :
Perform “Face to Face” visits as directed.Perform patient visits at the hospice Inpatient Unit Care Center and home setting.Provide clinical expertise to the interdisciplinary team regarding diagnosis and medical management of patient / family needs, ensuring that the highest quality of care is provided and that continuity and comprehensiveness are maintained.Palliative Care :
Collaborate with the attending physician to help palliative care patients to manage and plan for disease progression.Collaborate with referring physician concerning assessment findings, diagnostic, and therapeutic measures.Evaluate quality of life and responses to treatments.Perform Palliative Care consults to assess the patient, provide symptom management, educate regarding the disease process, coordinate providers and end-of-life planning, and link patients to available resources to enhance quality of life.Qualifications :
Education and Experience :
Master’s Degree in Nursing (MSN) from an accredited School of Nursing.Completion of a nurse practitioner program or certification as a Nurse Practitioner.Current Nurse Practitioner license in the State of Ohio.Current staff privileges at hospitals in the Agency’s service areas.Specialization in the area of Family Practice, Oncology, or Hospice is preferred.Knowledge of non-profit organizations is preferred.Minimum of five years of nursing experience; clinical nursing care in oncology, hospice, geriatrics, and intensive care.About Company
