Education and Experience:
- A bachelor of Social Work Degree from a social work curriculum accredited by the Council on Social Work Education is required. Master of Social Work preferred.
- Current Social Work licensure in the State of Ohio and the eligibility to obtain licensure in the State of Indiana or current Social Work licensure in the State of Indiana and the eligibility to obtain licensure in the State of Ohio required.
- Minimum of two years of experience in a clinical setting.
- Healthcare and/or Hospice experience preferred.
Responsibilities of Position:
- Completes a comprehensive initial and ongoing psychosocial assessment according to accepted standards of practice and agency policy for each patient and family or as requested.
- Assess patent and family members for risk related to abuse/neglect and suicide and those with active or past substance abuse and initiate contracts as deemed appropriate.
- Provide effective counseling approaches and processes for patients and families, including therapeutic and supportive interventions, education, life review, crisis intervention, and suicide prevention when indicated.
- Assist patients and families with end-of-life care planning including knowledge of living wills, DPOA, provision of care for dependents, assessing decision capacity, and funeral planning.
- Develop and implement a plan of care for the patient/family and attend IDG and care conferences as needed.
