Psychiatric Mental Health Nurse Practitioner - PMHNP at EverMind Where We Provide Better Solutions in Mental Health

In this role, you will :

• Collaborate with interdisciplinary care teams to provide comprehensive mental health services to residents of long-term care facilities.
• Conduct comprehensive patient assessments, develop individualized treatment plans, and provide ongoing psychotherapy and medication management.
• Document patient progress meticulously and maintain clear communication with referral sources, facility staff, and families.
