Job Summary
As a Permanency Specialist, you will play a vital role in collaborating with families and referral sources to develop comprehensive permanency plans. You’ll conduct written assessments of children and families, following the Statewide Adoption and Permanency Network (SWAN) protocol. Your primary responsibility will be identifying adoptive and kinship families needing additional support, facilitating assessments, in-home visits, and ongoing advocacy.
Key Responsibilities
- Implement SWAN units of service in line with protocol, including Child Profiles, Child Preparation Services, and Family Profiles.
- Collaborate with referral sources and families to create comprehensive permanency plans.
- Document attainable goals and objectives for each family member clearly.
- Provide informal advocacy and emotional support to families and children.
- Assist with transportation for families when needed.
- Offer follow-up support and engage in outreach activities.
- Maintain accurate documentation and adhere to all regulatory requirements.
- Respond to calls from resource families, caseworkers, and referral sources regarding SWAN activities.
- Meet productivity requirements based on units of service and maintain a flexible schedule for after-hours calls during on-call shifts.
- Participate in training for potential resource parents and provide ongoing education.
- Perform other duties as assigned.
Education and Experience
- Bachelor’s Degree in Counseling, Social Work, Psychology, or a related field preferred.
- Minimum of one year of related experience and/or internship; or an equivalent combination of education and training.
- Open to full-time candidates; part-time availability will be considered on a case-by-case basis.
Required Skills and Abilities
- Familiarity with social services compliance and training protocols.
- Ability to work flexible, non-traditional hours, including evenings and weekends.
- Proficiency in Microsoft Office Suite and general computer applications.
- Strong communication skills and professional etiquette.
- Excellent time management, multitasking, and initiative-taking abilities.
- Ability to build solid relationships with clients and demonstrate effective problem-solving skills.
- Valid PA Driver’s License, vehicle, and proof of adequate insurance coverage.
- Current Act 33/34 Clearances (within one year) and FBI Clearance.
Physical Requirements
- Use of general office equipment (computers, telephones, etc.).
- Ability to transport infants in car seats, requiring lifting and carrying up to 25 pounds.
