Community Integration Specialist provides aftercare services, discharge resources and family-based intensive support services to children, and their families, who are leaving residential treatment and transitioning to a community placement. This service works collaboratively with residential provider, child, and community placement provider to create an aftercare plan that provides support of the child’s personal adjustment in placement, school, community and with primary family relationships. The individual in this position will provide social and emotional support and education, community needs assessment and linkage, safety planning and assist in the development of natural supports.

This is a full time position, requiring both in-office presence and community-based hours when providing home visits.

Description

Essential Duties and Responsibilities include the following. Other duties may be assigned.

· Provide assessment of aftercare needs and develop aftercare plan, in collaboration with residential facility, prior to child’s discharge and coordinate services received by the youth and family while the youth is transitioning from residential treatment back into the home or community.

· Provide trauma informed in-home services to the child and placement provider, including psycho-education, case management, intensive safety planning, community service coordination, and regular home visits.

· Provide youth, family members, and care managers with community resource referrals in preparation for residents to leave ECH’s residential program and return to a community-based placement.

· Provide aftercare support and intervention services to youth and families, or other identified placement providers in compliance with the Children’s Division Aftercare contract.

· Participate in developing client-centered and permanency-focused aftercare treatment plans with measurable goals and objectives. Work closely with an interdisciplinary team and attend necessary meetings to facilitate continuity of care. Maintain effective communication and relationships with child’s team members.

· Provide support and education to parents, family members, or foster placement providers in preparation for residents to be discharged from the residential treatment program.

· Maintain a referral list of community-based supports to aid with actively linking patients to community resources to meet the patient's specific needs.

· Completes all required paperwork, attends all required meetings, trainings and supervision meetings as scheduled. Complete documentation of aftercare visits and progress notes in a timely manner.

· Driving will be required in this position. Hours will vary based on client needs, caseload size, and program needs.

· Maintain a trauma-informed, culturally sensitive, and client-responsive approach to service delivery. Responsible for being trauma aware, sensitive, and responsive to youth, families and colleagues through the completion of new hire and annual trauma informed education and training.

· Other duties as assigned

Education: Candidate will possess a bachelor’s degree in social work, psychology or a related area of study. Master’s degree is preferred.

Position requires the ability to sit, stand and walk. The position requires that the individual must interact with the public and must be able to speak and hear, answer the phone, and communicate pleasantly. This person must be able to drive with the ability and energy to visit more than one family per day.

Licenses: Must have a valid driver’s license in the State of Missouri[JA1] and reliable transportation.

Licensed, or provisionally licensed as a Counselor or Social Worker in the State of Missouri is preferred[JA2] . If not licensed, prefer four (4) years of experience in the social service arena. Other: Employee may have times of being on-call to support their clients during times of crisis. Must be willing and able to work in a varied, flexible routine which includes evening and weekend hours in order to accommodate client’s schedule.
