We need one great Provider: MD, Physician Assistant or Nurse Practitioner
Work Schedule: 5 Days a week, 8 AM to 4 Pm
Positions available: 1
Work Environment: Clinical-based setting
Position Summary: "Empowering and Enriching Lives"
Under the Medical Director's supervision, in a clinical-based environment, the Doctor/MD, PA, or NP provides primary care, performs assessments, develops care plans, treats participant complaints, educates participants and caregivers, and collaborates with the Interdisciplinary Team (IDT) to care for participants according to Every Age PACE standard.
Essential Duties and Responsibilities:
- Participates in meetings, communicates changes, and collaborates on care plans.
- Conducts assessments, exams, and integrates care plans.
- Manages illnesses and discharge planning within a clinical-based setting.
· Maintains records, handles coding, orders lab tests, and ensures safety.
Minimum Education:
- Bachelor’s Degree (Required)
- Doctor/MD: Doctorate Degree (Includes PhDs): MD or DO
- Board Certification – Internal Medicine or Family Practice
- Board Certification – Geriatrics preferred
- Current North Carolina physician license
- Current BCLS
- North Carolina certification and DEA Registration
- Physician Assistant: Graduate of certified or accredited Physician Assistant Program
- Nurse Practitioner: RN license and Nurse practitioner license with prescriptive authority preferred.
- CPR certification (Required)
Minimum Requirements:
- Two years’ experience as MD/Physician Assistant/Nurse Practitioner in a clinical-based environment geared towards geriatrics (Preferred)
