Established Home Health and Hospice company is seeking a compassionate, experienced Physical Therapist to provide in-home care in California. The ideal candidate will have at least one year of experience (preferably in home health), a California PT license, CPR certification, and reliable transportation. You will assess, treat, and educate patients to improve mobility and function, while maintaining accurate documentation. Flexible hours and professional growth opportunities are available.
Responsibilities:
- Deliver PT services as per physician’s care plan.
- Help patients with exercises, gait, and prosthetic training.
- Maintain timely clinical records.
- Participate in team meetings and ongoing education.
Qualifications:
- Licensed PT in California.
- Doctorate or Master’s in Physical Therapy.
- 1 year of PT experience.
- Current CPR certification.
- Valid driver’s license, insured vehicle.
