Summary:
The primary purpose of this position is to maintain the quality of patient care in accordance with the organizations objectives and policies through planning, coordination, and implementation and evaluation of the home health services provided. Additionally, the Clinical Manager is responsible to ensuring that all clinical staff are effectively trained, possess the necessary skills to conduct the visits assigned and that all work is completed and turned in according to agency policy and general nursing practices. Coordinates smooth operations within the agency. This is an exempt position under the FLSA.
Essential Duties and Responsibilities:
The essential functions include, but are not limited to the following:
- Coordinates care to patients, establishing a care plan from admission to transition to discharge which includes a comprehensive ongoing assessment of patients' needs.
- Review Start of Care (SOC), Recertification (Recert) and OASIS documentation, in full, to ensure proper service provision and utilization.
- Coordinate care between physicians, patients and field staff as applicable for each specific case.
- Review OASIS documentation for accuracy and appropriate Plan of Care (POC) development.
- Review and approve orders.
- Orient, develop, supervise, counsel and evaluate all nursing and therapy personnel, and provide daily supervision and guidance of nursing, therapy, and health aide staff to ensure the quality of services and maintenance of standards.
- Utilize independent judgment and knowledge to evaluate the quality and effectiveness of nursing and/or therapy services, analyzing appropriate data and information to identify opportunities in order to improve services and patient outcomes.
- Appraises staff performance and provide the education and counseling necessary to enhance the delivery of services and the professional development of nursing, therapy, and health aide staff.
- Answers all incoming calls regarding patient care.
- Provides patient care as needed.
- On occasion and when appropriate, communication with patient and/or patient families in relation to patient specific matters.
- Assist with case conferences as needed.
- Assure quality care is delivered as prescribed by the attending physician, code of nursing ethics and agency policy.
- Prepare delivery and/or coordinate staffing services education programs and staff meetings in areas requiring review and development.
- Periodically observe care given by personnel and document findings for employee evaluations.
- Assure patient assessment and care are within appropriate standards and the plan of care reflects such care.
- Review patients records for accuracy, eligibilities, current signed physician order, recertification, medical reviews, care plan, patient care conferences, supervisory visits, progress notes, etc.
- Assure documentation on patient records is timely, correct, within federal/state guidelines and agency policy. Assuring the agency is inspection ready at all times.
- Monitor referrals, assuring timely response, compliance to physician orders/preference.
- Instruct staff on policies/procedures.
- Communicate openly and maintain rapport with the Director of Nursing/Branch Director.
- Receive direction from the agency administrator on overall QAPI objectives for the agency.
- Be knowledgeable in data collection, data analysis methodology, and performance improvement methods needed to support and lead performance improvement projects.
- Identify opportunities for improvement through analysis of data, observation of operations, and consultation with leadership and staff.
- Collaborate with the quality committee and senior leaders to prioritize and develop QAPI efforts.
- Lead performance improvement projects and provide education and coaching in order to build needed skills in others to lead PIPs.
- Performs other duties as needed and assigned.

Education, Certification(s), and/or License(s) Required:
- Associate Degree in Nursing, Physical Therapy or Occupational Therapy.
- Five (5) or more years of nursing or therapy experience; preferably in a home health environment.
- Registered Nurse or Therapist licensure in Ohio or applicable multi-jurisdictional license.
- Current CPR Certification.
- Valid Drivers license.
