Job Summary
Overview:
We are seeking compassionate live-in in Rocky Hill, CT. 6 days or 7 days a week.
Join our team, Evolution Home Care, you will play a vital role in providing essential care and support to individuals in need within the comfort of their homes.

The ideal candidate will have experience working with people with disabilities, as well as a background in home health, group home, or assisted living environments. This role requires a strong commitment to patient care, observation, and maintaining a safe and supportive environment for clients.
Duties
- Provide personal care assistance, including bathing, grooming, and dressing.
- Assist clients with mobility and transfers to ensure safety and comfort.
- Monitor and document patient observations, reporting any changes in condition to appropriate healthcare professionals.
- Support clients with daily living activities such as meal preparation, medication reminders, and light housekeeping.
- Engage clients in social activities to promote mental well-being and companionship.
- Maintain confidentiality of client information in accordance with HIPAA regulations.
- Collaborate with family members and healthcare providers to create personalized care plans that meet the unique needs of each client.
Qualifications
- Experience in working with individuals with disabilities or in home health settings is preferred.
- Knowledge of dementia care practices is a plus.
- Strong observational skills and the ability to recognize changes in patients' conditions.
- Excellent communication skills and the ability to build rapport with clients and their families.
- Must be compassionate, patient, and reliable with a strong desire to help others.
