Overview
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. Duties
- Conduct comprehensive assessments of patients to develop individualized care plans.
- Coordinate and manage patient care across multiple settings, including primary care, emergency medicine, and specialized services.
- Perform diagnostic evaluations to determine patient needs and appropriate interventions.
- Collaborate with healthcare providers, patients, and families to facilitate effective communication and ensure continuity of care.
- Monitor patient progress and adjust care plans as necessary to achieve optimal health outcomes.
- Provide education and support to patients and their families regarding treatment options and resources available.
- Document all interactions and interventions in accordance with regulatory standards and organizational policies.
Requirements
- Must hold a valid Registered Nurse (RN) license in the state of practice.
- Proven experience in case management or related fields such as behavioral health or primary care.
- Strong knowledge of geriatric and pediatric patient needs is preferred.
- Excellent communication skills with a focus on patient service.
- Ability to work collaboratively within a multidisciplinary team environment.
- Strong organizational skills with attention to detail for managing multiple cases effectively.
