Role Overview

The Licensed Vocational Nurse (LVN) – Hospice is responsible for providing direct patient care under the supervision of a registered nurse (RN). This role focuses on end-of-life care, ensuring patients and their families receive compassionate support and symptom management. LVNs work closely with interdisciplinary hospice teams, following the plan of care and providing treatments, pain relief, and emotional support. Requires travel to patient homes across San Diego and surrounding areas. Both day shift and night shift positions are available.

Responsibilities:

The Licensed Vocational Nurse (LVN) – Hospice is responsible for providing direct patient care under the supervision of a registered nurse (RN). This role focuses on end-of-life care, ensuring patients and their families receive compassionate support and symptom management. LVNs work closely with interdisciplinary hospice teams, following the plan of care and providing treatments, pain relief, and emotional support.

This is a hybrid role that requires travel to patient homes across San Diego and surrounding areas. Both day shift and night shift positions are available.

Patient Care & Symptom Management

Provide direct hospice nursing care as defined by the State Nurse Practice Act.

Implement nursing interventions, symptom relief, and pain management strategies.

Assess patient needs, response to treatments, and medication effectiveness.

Patient & Family Education

Educate patients and families on hospice philosophy, symptom management, and self-care techniques.

Provide emotional and psychological support to families throughout the hospice journey.

Collaboration & Coordination

Work with an interdisciplinary hospice team, including physicians, RNs, social workers, and chaplains.

Participate in team meetings to ensure patient-centered care and continuous care plan adjustments.

Documentation & Compliance

Maintain accurate and timely clinical records that reflect the plan of care and patient condition.

Adhere to state licensure, hospice policies, and professional nursing standards.

Proficiency in completing medical documentation and following hospice protocols.

Education, Certification & Experience

Education: Graduate of an accredited Practical Nurse or Vocational Nursing program.

Licensure: Active Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) license in California.

Certifications: Current CPR certification required.

Experience:

Minimum three years of nursing experience (Community/home health or hospice preferred).

Familiarity with hospice philosophy and end-of-life patient needs.

Other Requirements:

Valid driver’s license with an insured and reliable vehicle.

Ability to meet physical demands, including prolonged standing and assisting with patient mobility.

Qualifications

1. A graduate of an approved school of nursing who is licensed as an LPN/LVN by the state of Indiana.
2. At least one year of recent nursing experience preferred.

Training:
Home health care experience preferred.
