Position Overview:
As a Medication Management LPN (Licensed Practical Nurse), you will play a crucial role in ensuring the safe and effective administration of medications for our clients. You will conduct in-home visits, educate patients and caregivers, and collaborate with healthcare professionals to enhance patient well-being.
Key Responsibilities of LPN (Licensed Practical Nurse) :
- Conduct in-home assessments to review and manage client medications.
- Educate clients and caregivers on proper medication usage, potential side effects, and adherence.
- Set up and organize medications, including pre-filling medication planners when necessary.
- Collaborate with physicians, pharmacists, and the care team to address medication-related concerns.
- Monitor clients for adverse reactions and report any significant health changes.
- Maintain accurate documentation of medication administration and client progress.
- Ensure compliance with healthcare regulations and company policies.
- Conduct thorough health assessments, including checking vital signs and implementing care plans.
Qualifications:
- Active LPN (Licensed Practical Nurse) license in the state.
- Experience in medication management, home health, or geriatric care preferred.
- Strong attention to detail and excellent communication skills.
