MDS Coordinator Responsibilities You'll Love :
Completing accurate MDS assessments per RAI coding guidelines
Initiating and maintaining care plans and supporting Interdisiplinary team as assigned
Creating and distributing monthly care plan calendars in a timely fashion
Monitoring & auditing clinical records, ensuring accuracy & timeliness
Coordinating optimal quality patient care together with the IDT
Protecting the confidentiality of Resident & Facility information at all times
Tracking and optimizing CMS 5 star ratings and quality measures
Closing out monthly billing timely and managing weekly utilization review meetings
Other duties and essential functions of the MDS coordinator role as assigned
What We Look For : Valid RN license.
Prior MDS 3.0 experience is required.
Previous long-term care experience is a must.
Ability to work 40 hours per week.
Highly organized, professional, and self-motivated.
Strong computer skills.
Excellent communication skills, both verbal and written.
Friendly, approachable, and a strong team player.
