I. JOB SUMMARY
Responsible for managing all aspects of the patient care coordination including orders, patient care reports into database and ordering supplies.
DUTIES & RESPONSIBILITIES
1. Handles all daily patient care coordination. Integration of services, whether services are provided directly or under arrangement, to assure the identification of client needs and factors that could affect client safety and treatment effectiveness
2. Coordination of care provided by all disciplines
3. Communication with the physician
4. Coordinated care delivery to meet each client’s needs, and to involve the client, representative (if any) and caregiver (s), as appropriate, in the coordination of care activities.
5. Provisions that each client and caregiver, where applicable, receive ongoing training and education from the home health agency regarding the care and services identified in the plan of care that the client and caregiver are expected to implement.
6. Training for each client and caregiver necessary for a timely discharge from the home health agency.
7. Documentation that reflects care coordination and ongoing communication within the interdisciplinary team
8. Ensures compliance with all state and federal referral/intake regulatory requirements.
9. Input received orders, reports, etc into patient’s record via database.
10. Order, stock and distribute supplies to staff as needed.
11. Assist scheduling as appropriate post receiving new orders to ensure procedures, lab draws, clients request, etc. are scheduled as ordered.
12. Provides client updates to third party payors as requested and notifies clinical supervisor and primary nurse of client authorization status.
13. Coordinates health services reimbursement. Tracks all agency health services clients obtaining authorization/certification for treatment from insurance/managed care companies.
14. Participates in quality assessment performance improvement teams and activities.

II. JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)
1. Registered nurse or Licensed Practical Nurse with current licensure to practice professional nursing in the state.
2. At least three to five years of experience in health care operations.
3. Demonstrated ability to supervise and direct professional and administrative personnel.
4. Ability to market aggressively and deal tactfully with customers and the community.
5. Knowledge of corporate business management, governmental regulations, and private payer practices.
6. Demonstrates good communications, negotiation, and public relations skills.
7. Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
