The Certified Home Health Aide assists the primary nurse / therapist in the provision of supportive care and comfort measures to home health and hospice clients as directed by the personal care service care plan.
Provides personal care services including bathing and other hygiene activities which enhance the client's physical comfort and well-being.
Takes and records vital signs as directed.
Recognizes and reports changes in the client's physical, mental and social status to the primary nurse and / or Clinical Director of Hospice.
Documents care and services provided during home visits in timely manner per policy and procedure.
