What are the responsibilities and job description for the Family Service Coordinator (PRP Case Manager) position at Excellent Healthcare & PRP Services?
We are currently seeking dynamic and motivated Family Service Coordinators (PRP Direct Care Counselors) for our PRP program to work with children, adolescents, adults, and families on behalf of one of our clients in Baltimore, MD.
We have immediate positions for those professionals who have backgrounds with mental health and/or substance abuse populations, have prior psychiatric rehabilitation and case management experience, and proficient with communication (verbal & written).
Primary Function:
The Family Service Coordinator is responsible for facilitating Community Integration, behavioral support services, and crisis intervention services to minors and adolescents, adults and families according to rehabilitation and therapeutic behavioral plans.
Job Expectations:
- Provide a minimum of 3 visits per month for minors clients and 6 times per month for adult clients.
- Plan to visit clients at home, office setting or in a variety of community settings.
- Complete client progress notes within a 24 to 48-hour time frame according to the goals on the Individual Rehabilitation Plan.
- Transport and participate in onsite psycho-education, social skills and/or wellness groups and/or activities at least monthly with clients.
- Provide collaboration with therapist monthly.
- Work cooperatively with family and other involved professionals to coordinate services with other agencies and programs in the best interest of the client
- Responsible for carrying out services and supports that meet the clients medically necessary mental health needs reflected in the client's individual rehabilitation plan (IRP) promoting independence, freedom of choice and overall wellness
- Assist the client with monitoring and obtaining access to all entitlement benefits for which the client may be eligible and monitor benefit status on a monthly basis helping the client to apply when necessary
- Be available to clients in the event of a mental health crisis while on duty
- Engaging assigned consumers in developing and maintaining therapeutic relationships.
- Assisting consumers in creating a daily structure that includes opportunities for developing skills necessary for functioning in the community.
- Implementing interventions identified on rehabilitation and therapeutic behavioral service plan.
- Developing and implementing therapeutic recreation, cultural enrichment, and independent skill development activities.
- Participating in the development and implementation of individual and rehabilitation service plans.
- Ensuring consumers are successful with managing their medication; and other direct service duties as assigned to promote the comprehensive and holistic well-being of youth, adults and their families.
- Facilitating the enhancement of an individual’s independent living and social skills, including the individual’s ability to make decisions about his or her life, while creating opportunities for choice regarding home, school or work, or community; and
- Promoting community resources to integrate the individual into the community.
- Participating in the Planning, Organizing and administering summer programs.
Skills/Abilities:
- Preferred experience in a fast-paced busy behavioral health or substance abuse environment.
- Demonstrates expertise in development and implementation of Individual Rehabilitation Plan
- Knowledge of state regulations and referral systems highly desirable.
- Demonstrates proficiency in the field of behavioral health.
- Demonstrates high level of proficiency in communication & written skills.
- Strong communication and counseling abilities.
- Ability to establish and maintain effective organizational and patient working relationships.
- Knowledge of community resources.
- Highly organized and driven in care coordination.
- Be able to be respectful and empathetic to consumers of various age groups, diversity and health condition.
Experience:
- Must have a minimum of two years direct care or related experience strongly preferred with dealing with at risk populations, including: children, adolescents, adults and families with serious emotional disabilities, severe mental health and co-occurring diagnoses.
- An Associate Degree is preferred.
- Must have valid driver’s license and be fully insured.
