Position: Director of Social Services
Location: Miami, FL
Key Responsibilities:
- Conduct comprehensive interviews with residents and families to gather detailed social histories and ensure personalized care plans.
- Collaborate with residents and families to develop and implement customized social service programs.
- Identify and address social, emotional, and environmental challenges, offering solutions like financial assistance, discharge planning, and coordination with community organizations for specialized services.
- Partner with the administration, medical, nursing, and other departments to design and implement comprehensive social services programs and engaging activities.
- Participate in facility surveys and inspections by authorized government agencies, ensuring compliance with quality standards.
- Advocate for residents by effectively communicating their social, psychological, and emotional needs to the care team, including physicians and medical professionals.
- Minimum of 3 years of experience in social services, preferably in a skilled nursing facility (SNF).
- Strong interpersonal, communication, and problem-solving skills.
