Salary: Up to $85,000/year

Adroit Care Rehabilitation and Nursing Center is seeking an experienced and compassionate Director of Social Services to join our team! If you're dedicated to improving the lives of residents in a supportive and collaborative environment, we want to hear from you!

Key Responsibilities:

• Conduct interviews with residents and families to gather comprehensive social histories.
• Develop and plan social service programs in partnership with residents and families.
• Address social and environmental challenges by identifying solutions, coordinating financial assistance, facilitating discharge planning, and working with community organizations for specialized services.
• Collaborate with administration, medical and nursing staff, and other departments to design and implement effective social service programs and activities.
• Actively participate in facility surveys and inspections by authorized government agencies as needed.
• Advocate for residents by communicating their social, psychological, and emotional needs to the care team, including medical staff and attending physicians.
  
  
  

Requirements:

• Minimum of 3 years of experience in social services, preferably within a skilled nursing facility (SNF).
• Strong interpersonal, communication, and problem-solving skills.
