Job Title: Director of Social Services

Location: West Palm Beach, FL

Facility: Beach Breeze Rehab and Care

Salary: Up to $78,000 per year

Key Responsibilities:

• Lead and oversee the Social Services Department, ensuring top-notch service delivery.
• Conduct comprehensive patient assessments, create individualized care plans, and support families during their transition into our facility.
• Develop and implement departmental policies, addressing any operational issues promptly.
• Regularly evaluate patient needs, maintain open communication with families, and engage them in social service program planning.
• Offer tailored solutions for social and environmental challenges, including financial assistance and community resource referrals.
• Work collaboratively with interdisciplinary teams to provide holistic resident care.
• Represent our facility during regulatory surveys and inspections, ensuring compliance with all standards.
• Serve as a liaison among residents, families, and medical staff, advocating for residents' social, psychological, and emotional well-being.
  
  
  

Requirements:

• Minimum of 3 years of experience in social services, preferably within a skilled nursing facility (SNF).
• Strong interpersonal, communication, and problem-solving skills.
