Job Description:
Arnold Walter Nursing and Rehabilitation Center is seeking a dedicated and experienced MDS Coordinator to join our team. The ideal candidate will conduct and coordinate the development and completion of the Resident Assessment (MDS) in accordance with current rules, regulations, and guidelines, including the implementation of RAPs and Triggers.
Key Responsibilities:
- Oversee the development and completion of MDS assessments, ensuring compliance with regulations.
- Maintain and periodically update policies and procedures for MDS and care plans.
- Perform administrative duties, including completing medical forms, reports, and evaluations.
- Develop and implement a quality assurance program for resident assessments and care plans.
- Collaborate with the Discharge Planning Coordinator on care plan portions of discharge plans.
- Participate in facility surveys and inspections by authorized government agencies.
- Monitor Quality Indicators (QI) and Quality Measures (QM) reports, implementing corrective actions as needed.
- Ensure timely completion and transmission of all assessments, reporting any issues to the Administrator.
- Assist with the coordination and scheduling of interdisciplinary care plan meetings.
- Collaborate with outside care team members, including residents' families and representatives.
- Assist in evaluating residents’ abilities and limitations, ensuring appropriate therapy and activities are scheduled.
- Contribute to therapy services and assist occupational therapists in carrying out activities.
- Must be a team player with strong collaboration and communication skills.
- Empathetic and professional when interacting with residents, families, and staff.
- Must demonstrate integrity and accountability in all work tasks.
- Strong initiative and problem-solving skills, with the ability to make informed decisions.
- 2 years of nursing home experience preferred.
