Job Title: MDS Coordinator

Facility: Bedrock Rehabilitation and Nursing Center at Winter Park

Location: Maitland, Florida

Shift: Full Time

Pay Rate: $39-$41 per hour

Key Responsibilities:

• Ensure Quality Care: Oversee care standards to ensure our long-term residents receive the highest level of support.
• Conduct Assessments: Perform detailed assessments and interviews for new residents to develop customized care plans.
• Lead Care Planning: Design individualized care plans with measurable goals for both short-term and long-term resident improvement.
• Collaborate Across Teams: Facilitate interdisciplinary meetings with healthcare professionals, families, and caregivers to create well-rounded care approaches.
• Community Engagement: Work closely with specialists to drive the best possible outcomes for our residents.
• Provide Leadership & Education: Lead nursing management meetings and deliver in-service training on MDS, RAPS, and Triggers, ensuring staff stay informed and proactive.
• Maintain Compliance: Keep the MDS/Care Planning schedule on track, focusing on non-Medicare residents.
  
  
  

What We’re Looking For:

• Licensed Registered Nurse (RN) in Florida
• At least 2 years of experience in a skilled nursing facility (SNF)
• Strong understanding of MDS, Medicare, and the care planning process
