Job Description:
Bedrock Rehabilitation and Nursing Center at West Volusia is seeking a knowledgeable and dedicated RN MDS Coordinator to join our clinical team. The MDS Coordinator plays a key role in the assessment and care planning process for residents, ensuring accuracy, timeliness, and compliance with all federal and state regulations.
Key Responsibilities:
- Complete and coordinate MDS assessments in accordance with regulatory requirements.
- Review and update resident care plans to reflect individualized needs and goals.
- Maintain accurate documentation, including forms, reports, and evaluations.
- Lead quality assurance programs related to MDS and care planning.
- Work closely with the Discharge Planning Coordinator to ensure smooth transitions of care.
- Participate in facility inspections and surveys.
- Analyze Quality Indicators (QIs) and Quality Measures (QMs) to identify and address trends.
- Ensure timely electronic transmission of assessments and communicate any delays to leadership.
- Collaborate with interdisciplinary teams and families to ensure continuity of care.
- Assist with resident evaluations, therapy coordination, and restorative care planning.
- Active RN license in the state of Florida.
- Minimum of 2 years’ experience in long-term care or skilled nursing preferred.
- Solid understanding of MDS 3.0, care planning, and regulatory guidelines.
- Strong communication, organizational, and leadership skills.
