Job Title: Temporary Social Service Worker

Location: Arnold Walter Nursing and Rehabilitation Center

Shift: 8:00 am - 4:00 pm

Duration: 4-6 months

Job Description:

Arnold Walter Nursing and Rehabilitation Center is seeking a dedicated and compassionate Temporary Social Service Worker to join our team. This role involves supporting residents' social and emotional needs by coordinating programs, planning discharge, and assisting in social services initiatives under the guidance of the Social Services Director.

Key Responsibilities:

• Assist in planning, implementing, and evaluating social service programs for residents.
• Maintain knowledge of federal/state regulations and recommend policy changes as needed.
• Participate in discharge planning, resident assessments, and social care plan development.
• Interview residents and families to gather social history and develop appropriate plans.
• Coordinate transportation and refer residents to community services when necessary.
• Educate residents and families on Medicare, Medicaid, and other financial assistance options.
• Collaborate with medical staff and other departments to address residents’ needs.
• Maintain progress notes and ensure they are detailed and informative.
• Assist with quality assurance and participate in government surveys/inspections as required.
• Facilitate and participate in relevant committee meetings, including Infection Control, Policy Advisory, and Quality Assessment.
• Ensure that resident and family wishes for end-of-life decisions (MOLST/POLST) are documented and implemented.
  
  
  

Qualifications:

• Experience: Minimum 1 years in a social services role; nursing home experience preferred.
• Skills: Strong communication, problem-solving, and interpersonal skills; ability to work in a collaborative team environment.
