Key Responsibilities:
- Assist residents with psychosocial needs, such as adjusting to facility life, providing emotional support, and aiding with discharge planning.
- Conduct assessments and collaborate on care planning for new and existing residents as part of the interdisciplinary team.
- Maintain accurate and timely documentation in compliance with state and federal guidelines.
- Advocate for residents' rights and help mediate any concerns or grievances from residents and their families.
- Coordinate social services interventions and referrals to external agencies (e.g., mental health, hospice, housing).
- Support discharge planning, including coordinating home services, equipment needs, and family or caregiver involvement.
- Actively participate in care conferences and collaborate with medical, nursing, therapy, and dietary staff.
- Plan and implement social programs and events to promote residents' mental and emotional health.
- Uphold confidentiality and advocate for resident dignity and respect at all times.
- High school diploma or equivalent required; associate or bachelor's degree in social work, psychology, human services, or a related field preferred.
- Prior experience in a healthcare setting, particularly long-term care or skilled nursing, is a plus.
- Knowledge of federal and state regulations related to resident care (e.g., OBRA, HIPAA).
- Strong communication, documentation, and organizational skills.
- A compassionate, patient, and supportive demeanor.
Salary : $15 - $19
