Excelsior Family Medicine is seeking a Community Health Worker to join our team. As our Community Health Worker, you will be responsible for helping our patients gain community access to health insurance, food, housing, health information, community programs, and quality care. By building strong relationships with our patients and the community, you will directly contribute to the development of culturally competent and accessible healthcare programs, ultimately improving health outcomes and reducing healthcare disparities in underserved populations. We are looking for a community minded individual to help ensure integrated care while partnering with health organizations and clinical staff. Your knowledge of current community programs will be imperative to developing and maintaining effective programs.
To Qualify:
- High School Diploma required, BA in counseling or related field preferred.
- Community Health Worker certification.
- Registered Agency Affiliated Counselor Credential or ability to become registered within 60 days required.
- Comprehensive knowledge of community resources and public systems.
- Minimum three years of working with youth and families.
- Exceptional communication and interpersonal skills.
- Must have strong multitasking and organizational skills.
- Ability to work in a fast-paced environment and make ethical and safe decisions.
- Ability to manage individual stress and perform essential job functions in a safe and professional manner.
- Minimum 21 years of age.
- Valid driver's license and acceptable driving record in accordance with organizational standards.
- Must be able to pass a criminal background in accordance with organizational standards.
Core Responsibilities Include:

- Advocating for the needs of individuals and the community by partnering with healthcare teams and individuals to meet their unique needs.
- Educating individuals and communities about how to use health and social services systems.
- Completing personal and individualized health assessments to identify specific health needs and concerns.
- Assist individuals and families access community resources, complete applications, and forms, and complete checks on individuals with specific health conditions.
- Participating in care coordination and/or case management
- Conduct health promotion and disease prevention education to individuals and groups to better understand their health conditions and find ways to develop a healthier lifestyle.
- Facilitate referral services by providing guidance on available healthcare resources and social services, making referrals based on specific needs of community members.
- Demonstrate cultural competency by understanding and respecting diverse cultural practices and beliefs prevalent within the community to deliver care in ethical and culturally sensitive terms.
- Build partnerships with local organizations, schools, and businesses to create a comprehensive network of support for community health initiatives.
- Complete administrative tasks on an ongoing basis to ensure organized and proper record-keeping is achieved.
