What are the responsibilities and job description for the SUDP position at Excelsior Wellness?
Excelsior Family Medicine is an established clinic of 10 providers working in a cross-disciplinary team to address client needs. Our dedicated providers encourage clients to engage in mental and physical health, and we are proud to support this with the right care at the right time.
As a member of the provider team, your role as a Substance Use Disorder Professional (SUDP) offers expertise not only to clients under your care but also to a cross-disciplinary provider group. You will work with support staff who will assist you with the flow of care by printing letters, making phone calls, and other administrative tasks.
Required Qualifications:
- High School diploma or GED.
- Hold a current Washington State Department of Health License as a Substance Use Disorder Professional (SUDP).
- Excellent communicator in written and verbal form.
- Experience working with adolescent clients as an SUDP or SUDPT.
- Experience functioning within a multi-disciplinary team including mental health counselors and medication management providers.
- Experience working in an IOP or Co-IOP program setting.
- Ability to pass a criminal background check.
- Valid driver's license and acceptable driving record, per agency standards.
- Must be at least 21.
Core Responsibilities:
- Maintain a SUD Treatment Caseload of 14-16 clients, providing personalized care.
- Conduct comprehensive SUD assessments to tailor personalized treatment plans.
- Conduct SUD group therapy within the EXIST Co-IOP program
- Engage in community outreach to promote awareness and support for those affected by substance use disorders.
- Serve as a subject matter expert, handling inquiries and developing action plans to address them and assisting with the preparation and dissemination of communications.
- Support EXIST CO-IOP programs within Excelsior Family Medicine
- Facilitate SUD education groups for the external community, empowering them with knowledge for prevention and intervention.
