Are you a Licensed Practical Nurse (LPN) looking for a rewarding opportunity in homecare? Our growing agency is expanding services into Monson, South Hadley, Hadley, Easthampton, Northampton, Palmer, Three Rivers, and surrounding areas, and we’re looking for compassionate and dedicated LPNs to join our team!
Position Overview:
As a Homecare LPN, you will provide personalized nursing care to patients in their homes, supporting their health and independence. You’ll play a vital role in monitoring patient conditions, administering treatments, and providing education to patients and their families.
Responsibilities:
- Provide skilled nursing care in accordance with the patient’s care plan.
- Monitor and document patient health status, including vital signs and symptoms.
- Administer medications, wound care, and injections as prescribed.
- Educate patients and caregivers on disease management and home care techniques.
- Collaborate with RNs, therapists, and other healthcare professionals to ensure quality care.
- Follow infection control and safety protocols to protect patients and staff.
Requirements:
- Must have a valid LPN license "In Good Standing" in Massachusetts.
- Must have a valid CPR Certificate.
- Must pass a criminal background check.
- Homecare Experience Preferred, but not required - willing to train!
- Reliable transportation to travel within assigned areas.
- Familiarity with Axxess EMR software is a plus.
- Strong communication and time-management skills.
- A patient-focused, compassionate approach to care.
- Must be available to work every other weekend.
