Our growing homecare agency is expanding into Monson, South Hadley, Hadley, Easthampton, Northampton, Palmer, Three Rivers and other surrounding areas . We are seeking compassionate PTs to join our team!
Position Overview :
As a Homecare Physical Therapist , you will provide one-on-one rehabilitative care to patients in the comfort of their homes. You'll help improve mobility, strength and independence, enabling our patients to regain their quality of life.
Responsibilities :
- Evaluate patients' conditions and develop individualized treatment plans.
- Provide hands-on therapy, exercise, and education to support recovery and prevent decline.
- Work closely with a multidisciplinary team to ensure optimal patient outcomes.
- Document patient progress and update treatment plans accordingly.
- Educate patients and caregivers on home exercise programs and injury prevention.
