Self-Direction DSP - New Winsor Area (Job #KC-RA)
Exceptional Family Resources is seeking a Community Habilitation Professional to work in the New Windsor, NY area providing community habilitation to a young woman whose interests include fashion, music, and art.
With the support of her family, she is looking for individuals who are reliable, patient, and caring
to provide her with support and assistance to learn and build skills with money management, independent living skills, safety skills, and parenting skills. She relies on the assistance and education to help her to support her child. She needs to learn to be independent for both herself and with her parenting skills. She is a person who takes some extra time to open up and needs to have the support of someone who will be compassionate and take the time to get to know her and her interests.
DSP's will also be responsible for providing transportation to and from community outings. Prior experience in the field of developmental disabilities is a plus but not required. Background check, fingerprinting, and DMV record check will be conducted prior to hire by the non-profit agency, along with general initial training. Specific training regarding this individual will be provided by the family with ongoing support provided by the Support Broker involved with the person's self-directed services.
General responsibilities will also include:
- Follow individual's staff action plan/daily activity schedule.
- Complete all necessary paperwork as needed and in a timely manner to submit for timesheets, reimbursement, and daily service notes
- Must understand and comply with all HIPAA related guidelines
- Complete all required trainings when they are due
Flexibility in scheduling can be requested and will be between the family and the staff; current available shifts are flexible and to be discussed with the family. Evening and weekend availability may be required. The payrate is $32/hr with reimbursement for mileage and staff's fees toward participation in service activities; these will be paid for within the self-direction budget limits.
