As an LPN, you will provide direct care to residents by monitoring their condition, administering medications, and performing necessary tasks such as taking vital signs and documenting care. You will collaborate with the interdisciplinary team to develop and implement care plans that enhance the physical, social, and mental well-being of residents, ensuring that all care is compliant with regulatory standards. Our LPNs also assist with staff training, participate in quality assurance activities, and maintain clear communication with residents, families, and medical staff about changes in care.
Schedule varies: Both full-time and part-time positions available; Shift hours are 6:30am-6:30pm or 6:30pm-6:30am. Day shift is Fri/Sat/Sun; Night shift is 3 weekdays with no weekends. Holidays required.

- Must be 18 years of age or older
- Must have an active LPN license in the state of Indiana
- At least one-year of related experience is preferred.
