Job Type Full-timeDescriptionJob Summary : The Home Visitor develops a relationship with the family and works with the family to build upon strengths and to enhance self-sufficiency. Implement Nurturing Parenting Curriculum through weekly home visitation, while supporting the family with their day to day struggles. Assess child developmentally and parent in the progress of their goals. Communicate effectively with referring parties and help connect clients to outside agencies that may meet their needs. Document interactions and report as necessary.Essential Functions :
- Develop a relationship with families and work with the family to build upon strengths and to enhance self-sufficiency.
- Maintain and manage a case load of approximately fifteen (15) to twenty (20) families.
- Utilize community resources; conduct in-home teaching and supportive problem solving to help families meet the goals of their Family Service Plan, which is to be reviewed with the family on a quarterly basis.
- Make regularly scheduled home visits on all families assigned in accordance with the level of intensity based on the need as determined by the assessments, home visitor observations and feedback from other support staff. Document each home visit on a weekly home visit form, which is to be maintained in the family case file.
- Keep accurate data and records on all families served to include : Assessments, screenings, reviews, Nurturing Parenting lessons / homework and ongoing documentation of conversation.
- Assist in all aspects of the LAP / HV program as necessary.
- Be familiar with EPU policies and procedures and maintain proper confidentiality.
- Position requires data entry, answer phones and various computer activities.
- Position requires local travel on a daily basis.
- Keep an accurate mileage log when utilizing own vehicle for home visits, meetings and / or training.
- EPU facility hours are Monday - Friday; 8 : 30 AM - 5 : 00 PM; with some exceptions based on family's needs. May be required to work evening to support other program needs.
- Must perform other duties as they are assigned by the Director or President / Chief Executive Officer.RequirementsKnowledge, Skill and Experience : Education : Must have a Bachelor's Degree in Child Development, Psychology, Social Work or related field. May consider work experience of 2 year or more in lieu of education.Skills : Must have strong communication skills written and verbal, planning / organizing, information gathering, identifying solutions, summarization, listening comprehension, goal setting, multi-tasking, teamwork, and collaboration, initiative and able to make independent decisions within guidelines, code of ethics.Experience :
- Experience working with families whose children at risk for abuse and neglect by supporting and improving parental capabilities and child development.
- Case management and a strong understanding of community resources required.
- Must have a warmth and sensitivity for working with children and their families.
- Must be able to work with a diverse population and be a team player.
- Must have the capacity to engage the family and yet maintain the boundaries of a helping relationship.
- Good problem solving and communication skills and are able to work with families to empower them to engage in problem solving.
- Must hold valid California Driver's license and good driving record with no serious violations for the previous two-year period and be able to provide proof of current automobile liability insurance.
- Must have reliable transportation in order to visit families in their homes.
- Bi-lingual in Spanish is a plus.
- Computer literate, knowledge of Microsoft Office products.Physical Demands : Typical Working Condition : Staff is required to visit families in their homes. EPU does not have control over the location and condition of all the environments staff may be exposed.Equipment Used : Personal vehicle for local travel, computers, printers, telephones, and fax machines.Essential Physical Tasks : Working with children and their families may require the ability to sit on floors, stand, stoop, crawl, lift, carry, push and pull.This position description may not be an exhaustive list of all duties, knowledge, or abilities associated with this classification.
