Job Summary: 

The Bright Beginnings Case Manager - Facilitator (BCM-F) works within the vision, mission and philosophy of the agency to provide families and children with support, resource coordination, case management, and evidenced based or informed early childhood interventions as identified in the Plan of Care. Working within the clinical team, develops a strength based relationship with the child and caregiver, organizes and facilitates groups or individual interventions for children and their caregivers incorporating concepts of resiliency and recovery in support of child and family wellness and self-sufficiency. The BCM-F may provide services under multiple contracts documenting and submitting data accordingly.


Essential Functions:

  • Administer The Child and Adolescent Needs and Strengths (CANS) structured assessment tool, the Pediatric Symptoms Checklist-35 (PSC-35) psychosocial screening tool, and other validated tools as required by the program, to identify child and family needs and useful strengths. Able to use these as a framework to inform planning, support decisions, and monitor outcomes for the child and family. 
  • Provide case management to identify resources to needed services and monitor the progress of the child (the client) as outlined in the Plan of Care. Case management involves providing linkages, monitoring progress, advocating, brokering or ensuring access with or on behalf of the client. 
  • Monitor the needs of the child, caregiver and the family and support them as a unit.  
  • Work with families and others involved with the child in developing and implementing behavioral, trauma-informed and psychoeducational interventions as specified in the Plan of Care.
  • Provide rehabilitative services (rehab) aimed at restoring, improving and/or preserving the child’s functional, social, communication, or daily living skills to enhance self-sufficiency or self-regulation relevant to the developmental stage of each child.  
  • Assist families in dealing with stressors substantially affecting the wellbeing of the child. 
  • As needed, provide psychoeducation, collateral services and/or training to the caregiver(s) present in the child’s life to increase resiliency, recovery and utilization of services that further improve child’s functioning and healthy development.
  • Follow up to ensure families have ability to access and obtain appropriate services within the Center as well as in the community.
  • Plan and facilitate evidenced based group and/ or individual interventions based on child’s plan of care.
  • Obtain and maintain current community resource information.  
  • Develop and maintain cooperative relationships with community agencies/ providers serving young children and their families.
  • Accurately document client services and submit corresponding progress notes, data & billing sheets, adhering to the Center’s established protocols. Able to use electronic health record system or other data system as required. 
  • Communicate with assigned families explaining Center services and procedures. 
  • Provide the family with appropriate support, including but not limited to childcare and transportation needed to attend center or other community service appointments.
  • Able to cultivate and provide culturally responsive care to children and their families accessing services at the Center.
  • Represent EPU in a professional manner to include demonstrating sound problem-solving skills in carrying out all responsibilities of the position including when consulting over the phone or face to face while serving in an afterhours “standby” capacity.
  • Promote positive work environment through collaboration, consultation, problem-solving and participation in various team building activities.
  • Maintain, manage, and process client records and information in a confidential manner that meets HIPAA and other applicable confidentiality regulations. 
  • Seek out reflective practice, supervision and consultation to ensure adherence to ethical, evidence-based and /or informed practices. Have willingness to learn and implement new child and/ or family evidence-based, trauma informed interventions based on program design and requirement. 
  • Adhere to Fresno County managed care regulations as it relates to provision of services and its documentation and billing requirements.
  • Position requires flexibility of hours outside of standard operating hours of Monday through Friday, 8 – 5 pm, based on program funding and position needs. Some evening and weekend work will be required.
  • Achieve annual unit of service and/or other performance goals established by your supervisor.
  • Maintenance of certificates and job related tools, resources, and toys.
  • Adhere to EPU standards, policies and procedures.
  • Perform other duties as assigned by your immediate Supervisor, Manager of Clinical Services and/or the Chief Executive Officer.

Requirements

Knowledge, Skill and Experience:

Education: Bachelor’s Degree in Psychology, Child Development, Social Work or closely related field.


Skills: Must have strong communication skills written and verbal; able to make good, logical independent decisions within guidelines, accept responsibility; be detailed oriented, dependable, reliable, committed, highly motivated, and adaptable; have self-confidence, strong time management skills and a professional code of ethics and professional demeanor; and take pride in producing a high-quality product. 


Experience:

  • 2 years’ full-time experience working as part of a multidisciplinary team in working with children and families providing early childhood intervention services for children with social/emotional, developmental, and behavioral needs. 
  • Experience in obtaining confidential sensitive information in a compassionate manner.
  • Experience in collaborating with staff, professionals, and families in developing and implementing family-centered Plan of Care.
  • Experience in handling client records and information confidentially and in accordance with HIPAA and other applicable regulations.
  • Experience in providing evidence-based interventions to children and families.
  • Experience in working with diverse populations.
  • Experience in developing, organizing, and providing parent education groups and/ or individual evidenced based interventions in accordance with Plan of Care recommendations. 
  • Experience in working as part of a team in implementing new and existing systems that promote both quality and efficiency. 
  • Must be computer literate with Microsoft Office programs such as Word, Excel and Outlook. 
  • Knowledgeable of community resources and services available to families/young children.
  • Hold valid California Driver's license and good driving record with no serious violations for the previous two-year period and be able to provide proof of current automobile liability insurance.

Physical Demands:

Typical Working Condition: Staff is required to visit families in their homes. EPU does not have control over the location and condition of all the environments staff may be exposed.

  

Equipment Used: Motor vehicle for local travel, computers, printers, telephones, and fax machines.


Essential Physical Tasks: Working with children and their families require the ability to sit on floors, reach, twist, turn, kneel, stand, stoop, crawl, lift, carry, push and pull.
