Job Summary:
The Case Manager is responsible for engaging and providing case management for up to 25 families.
Essential Functions:
- Maintain and manage a case load of up to twenty five (25) families.
- Make regularly scheduled home visits for all families assigned in accordance with the level of intensity based on the need as determined by the assessments, Child Welfare Service (CWS) referral and other support staff. Document each home visit on a weekly home visit form, which is to be maintained in the family case file.
- Develop a relationship with families and work with the family to build upon strengths and to enhance self-sufficiency.
- Work with family to develop and monitor individualized case plan.
- Utilize community resources; conduct in-home teaching and supportive problem solving to help families meet the goals of their Family Service Plan, which is to be reviewed with the family on a quarterly basis.
- Facilitate one on one Nurturing Parenting curriculum in the home when appropriate.
- Attend Team Decision Meetings (TDM) as per the request of Child Welfare Social Worker.
- Make regularly scheduled home visits for all families assigned in accordance with the level of intensity based on the need as determined by the assessments, Child Welfare Service (CWS) referral and other support staff. Document each home visit on a weekly home visit form, which is to be maintained in the family case file.
- Facilitate various community educational classes for families as part of the NRC.
- Conduct ASQ & ASQ-SE screenings for families with children ages 0-5 & follow-up with any necessary referrals.
- Connect families with to Neighborhood Resource Center (NRC) services and pertinent outside agencies as needed.
- Prepare and keep accurate case notes.
- Assist in all aspects of the LAP program as necessary.
- Be familiar with EPU/VDC policies and procedures and maintain proper confidentiality.
- Maintain, manage, and process records, files and information in a confidential manner that meets HIPAA and other applicable confidentiality regulations.
- Utilizing own vehicle for home visits, meetings and/or training.
- VDC facility hours are Monday – Friday; 8:30 AM – 5:00 PM; with some exceptions based on family’s needs.
- Must perform other duties as they are assigned by the Director, Manager or President/Chief Executive Officer.
Requirements
Knowledge, Skill and Experience:
Education: Bachelor’s degree in Social Work or related field; a minimum of 60 units of completed college credit and 2 years of experience in providing direct service to families; or 5 years case management experience providing direct services to families may be substituted.
Skills: Must have strong communication skills written and verbal, planning/organizing, information gathering, identifying solutions, summarization, listening comprehension, goal setting, multi-tasking, teamwork, and collaboration, initiative and able to make independent decisions within guidelines, code of ethics.
Experience:
- Experience working with families whose children at risk for abuse and neglect by supporting and improving parental capabilities and child development.
- Case management and a strong understanding of community resources required.
- Requires data entry and case note writing skills.
- Must have a warmth and sensitivity for working with children and their families.
- Must be able to work with a diverse population and be a team player.
- Must have the capacity to engage the family and yet maintain the boundaries of a helping relationship.
- Good problem solving and communication skills and are able to work with families to empower them to engage in problem solving.
- Must hold valid California Driver's license and good driving record with no serious violations for the previous two-year period and be able to provide proof of current automobile liability insurance.
- Must have reliable transportation in order to visit families in their homes.
- Bi-lingual in Spanish is a plus.
- Computer literate, knowledge of Microsoft Office products.
Physical Demands:
Typical Working Condition: Staff is required to visit families in their homes. EPU does not have control over the location and condition of all the environments staff may be exposed.
Equipment Used: Personal vehicle for local travel, computers, printers, telephones, and fax machines.
Essential Physical Tasks: Working with children and their families may require the ability to sit on floors, stand, stoop, crawl, lift, carry, push and pull.
Salary : $24 - $25
