Core activities of the role
· Perform the medical history and physical examination for patients of all ages.
· Personalize the visit to the reason for the clinic encounter (acute issue, chronic issue, or health maintenance).
· Appropriately utilize and interpret medical histories, physical exams, and diagnostic tests to develop the best treatment plan.
· Implement and manage treatment plans including prescribing medications, performing procedures, and other treatments as necessary.
· Coordinate referrals to other healthcare professionals and medical facilities.
· Counsel and educate patients about their tests, medications, treatment plans and how to make lifestyle changes to improve their health
· Be familiar with and utilize community resources to assist patients with their physical, mental, and social needs.
· Work with colleagues to ensure that Exemplar Care patient needs are addressed in a timely manner.
· Timely documentation of clinic encounters and patient interactions in the electronic health record.
· Adhere to the policies and procedures of Exemplar Care.
· Adhere to government rules, regulations, and laws.
· Other duties as assigned

Required Qualifications (Education and Experience)
· At least 1 year's related experience as a provider
· Current NP/PA certification required in the state of practice and eligible for any other state
· Current PALS certification required within 30 days
· Competence to practice to the fullest extent allowed by your state licensure.
Physical Requirements
Prolonged periods of sitting at a desk and working on a computer.
Must be able to push, pull, lift, lower, or other movements at 25 – 50 pounds at rare times with or without assistance
Must be able to push, pull, lift, lower, or other movements at 10 – 25 pounds at semi-often times
Must be able to push, pull, lift, lower, or other movements at 0 – 10 pounds frequently
Enviromental Requirements: Moderate levels of sound, consistent amount of moving around between rooms required, in a temperature-controlled environment, and consistently working with the general public.
