What are the responsibilities and job description for the Community Navigator position at Exodus Recovery Inc.?
Exodus Recovery, Inc is looking to hire full-time Community Navigators for day shift and night shift for our new Santa Monica Bridge Program.
POSITION SUMMARY:
The Community Navigator (CN) is responsible for helping complex clients to navigate and appropriately access community services and resources. The CN works alongside the treatment team, taking an integrated approach to access community services and provide care management to clients in interim housing program. The CN will provide client advocacy, social support, case management and provide linkage and referral services under the direction of the Program Director, Mental Health Clinicians and Exodus Leadership.
MINIMUM QUALIFICATIONS:
- Extensive lived experience and/or professional experience in working with complex, homeless populations who are high users of multiple public health systems.
- Two (2) years clinical experience in health sciences or related field. Strong verbal and written communication skills. Ability to work independently and show mature judgment. Will be screened for driving competency through the human resources onboarding process.
EDUCATION:
- High School Graduate or GED equivalent.
- Valid California driver’s license and a good driving record is required.
- Must be 21 years or older to drive. Minimum 3 years driving experience.
- Peer Advocate or Community Health Worker certification preferred.
Commensurate with experience.
