Job Description
Reporting directly to the Director of Nursing, this licensed, compassionate and technically savvy individual delivers the best possible patient care by caring for ill, injured, convalescent, or disabled persons in one or more designated facilities. Acts as a liaison between the facility, their residents and the marketing team helping to build the relationship and identify opportunities to deliver patient care. Licensing required.
Position Functions
• Under the direction of the RN assists in identifying the patient’s physical, psychosocial and environmental needs as evidenced by documentation, clinical record, case conference, team report and evaluations.
• Participate in planning, implementing care in conjunction with the RN, according to the plan of care.
• Observe patients, charting and reporting pertinent changes in patients’ conditions, responses to interventions, treatments, and taking any necessary action including contacting doctors and other authorized personnel as appropriate.
• Perform on-call responsibilities as assigned
• Respond to patients’ family/ caregiver’s calls and determine how to assist them
• Work as part of a health care team to educate facility residents and staff on program offerings, assess patient needs, plan and modify care and implement interventions
• Collect laboratory samples as ordered
• Prepare patients for examinations, tests or treatments and explain procedures as needed
• Perform skilled nursing procedures like catheter insertion/change, suture removal, ostomy and wound care
• Evaluate nursing intervention outcomes, conferring with other health care team members as necessary
• Instruct patient/caregiver family members regarding health promotion and maintenance in the home setting
• Maintain HIPAA compliance at all times
• Submit necessary reports and required patient care documentation timely
• Meet mandatory continuing education requirements of the agency/licensing board
• Demonstrate commitment, professional growth and competency
• Promote Agency philosophy and administrative policies to ensure quality of care
Qualifications
Soft Skill Requirements
• Excellent communication skills, both oral and written.
• Compassionate and caring attitude and the ability to deal with people from a wide range of backgrounds, ages and temperament.
• Ability to use discretion concerning sensitive information and adhere to HIPAA.
• Ability to work on own initiative within clearly defined parameters.
• Ability to carry out designated tasks including reporting outcome of actions taken.

• Easily adapts to changes in workload
• Customer service abilities with good listening skills
Education and Experience
• Nursing License
• Experience using required equipment like hypodermic needles, nebulizers and accessories, patient care beds and accessories for specialty care, fall prevention devices and accessories, Spiro meters and accessories/supplies
• 2 years of experience dealing with disabled or elder care
• Knowledge and experience of relevant software applications for scheduling and reporting requirements
• Proficient in spelling, punctuation, grammar and other English language skills
• Bi-lingual in Spanish a plus
