Pay Rate : $18 / hr on W2
We are looking for a Health Management Assistant to join one of our Fortune 500 Healthcare clients.
Responsibilities :
- Health Coaching / Intervention (50%) : Provide health coaching to members via phone or in person, document patient information accurately.
- Program Development & Monitoring (30%) : Identify areas needing health programs, monitor quality indicators, and create educational materials for members / providers.
- Implementation Support (10%) : Help implement health management activities, facilitate programs for medium-risk patients, and analyze reports.
- Strategy Development (10%) : Develop strategies to enhance program participation and effectiveness, maintain communication with providers, representatives, and members.
