This position facilitates and coordinates long-term services and supports (LTSS), utilizing a person-centered approach to improve psychosocial and health outcomes for its participants. In accordance with procedures and requirements of CHC Managed Care Organization, the Service Coordinator (SC) has responsibility for ongoing assessment, service planning, service coordination, monitoring, follow-up, and evaluation for persons with disabilities and/or older adults. The SC works within a collaborative team approach to identify and address participants’ needs, preferences, and goals related to physical health, behavioral health, social services, and long-term services and supports.
Although this position involves travel to conduct in person assessments, it will require an individual to work remotely as well. Staff will be responsible for a caseload of approximately 75 participants within the Lackawanna County.
Duties and Responsibilities:
- Actively coordinates with the participant and service providers to ensure seamless coordination between physical, behavioral and support services
- Assist participants in obtaining Home and Community Based Services that will promote independent living
- Assist the participant in identifying and choosing willing and qualified providers
- Collect additional necessary information including the participant’s preferences, strengths, and goal in order to work towards the development of a Person-Centered Service Plan. PCSPs should be conducted on annual basis or more frequently if needed.
- Explore coverage of services to address the participant’s identified needs through other sources, including services provided under Medical Assistance, Medicare or private insurance and other community resources
- Identify, coordinate and assist participants in gaining access to needed LTSS and Medical Assistance services, as well as non-Medicaid funded medical, social, housing, educational, and other services and supports
- Inform participants about available LTSS, required assessments, the Person-centered service planning process, service alternatives, service delivery options including opportunities for Self -direction, roles, rights including DHS Fair Hearing rights, risks and responsibilities, and to assist with fair hearing requests when needed and requested, and to protect a participant’s health, welfare and quality on on-going basis
- Lead the Person-Centered Service Planning (PCSP) process and oversee the implementation of PCSPs
- Continuously monitor LTSS needs for participants
- Work with the participant to complete activities necessary to maintain LTSS eligibility
Qualifications:
Registered Nurse or have a Bachelor’s degree in Social Worker, Psychology, OR another related field with at least three (3) years of experience in a social service or healthcare related setting.
Job Skills:
- Three years working in social service or health care related setting
- Familiarity with Person-Centered Service Planning
- Experience working with people with disabilities or seniors in need of LTSS
- Knowledge of the home and community-based service system
- Experience conducting LTSS needs assessments and monitoring LTSS delivery
- Cultural competency and the ability to provide informed advocacy
- Ability to interact with physicians and other health care professionals in a professional manner is required
