Overview
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality nursing care to patients in a home setting.
13 week contract- Home Health LPN.Must have at least 1-year of recent home health experience. Hands-on HCHB experience required. HCHB requirement may be waived if candidates have at least 7.5 years of recent home health experience as well as the experience outlined below:Must have experience with Microsoft office including email and teams. Must have experience with Trach care, Wound VAC and Wound Care such as Unna Boots/Coban, Peripheral lab skills, nephrostomy skills, and peg tube skills. $1400/week plus .42 a mile for patient related travel.
Responsibilities
- Administer medications and treatments as prescribed.
- Perform patient assessments.
- Monitor patient vital signs and report any significant changes to the healthcare team.
- Provide support in emergency situations, including gastric lavage and ER procedures.
- Maintain accurate patient records using electronic health records systems such as Cerner.
- Collaborate with healthcare professionals to develop and implement patient care plans.
- Educate patients and their families on health management and wellness strategies.
Qualifications
- Valid LPN license in the state of practice.
- Proficiency in triage procedures and emergency response protocols.
- Familiarity with catheterization techniques and sterile processing standards.
- Knowledge of neonatal care, including operation of neonatal ventilators, is a plus.
- Strong communication skills with the ability to work effectively within a team environment.
- Compassionate demeanor with a commitment to providing exceptional patient care.
- Previous experience in hospice care is advantageous but not required.
