Overview
We are seeking a dedicated and compassionate Registered Nurse (RN) for our Home Health division. The ideal candidate will possess a strong background in nursing with a focus on providing high-quality care to patients in their homes. This role requires excellent clinical skills, the ability to work independently, and a commitment to enhancing patient outcomes. The RN will be responsible for assessing patient needs, developing care plans, and delivering nursing care in accordance with established protocols.
13 week contract- must have at least 12 months of home health experience within the last 3 years.
OASIS is Required and HCHB is strongly preferred.
Monday-Friday, 40 hours a week
On- Call twice a month and there is a weekend rotation of every 3rd weekend.
$2,000/Wk plus .42 a mile for patient related travel
Responsibilities
- Conduct comprehensive assessments of patients' health status and needs in their home environment.
- Develop, implement, and evaluate individualized care plans based on patient assessments.
- Administer medications and treatments as prescribed by physicians.
- Monitor patients' vital signs and overall health, documenting changes and communicating with healthcare providers as necessary.
- Provide education to patients and families regarding health management, treatment plans, and medication administration.
- Utilize electronic health record systems such as Cerner for documentation and communication of patient information.
- Collaborate with interdisciplinary teams to ensure comprehensive care delivery.
Requirements
- Valid Registered Nurse (RN) license in the state of practice.
- AR least 12 months of home health experience within the last 3 years
- OASIS is required and HCHB is strongly preferred
- Ability to ventilate patients when necessary and manage complex medical equipment.
- Excellent communication skills and the ability to work independently.
