Job Summary
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health team. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. This role requires a strong commitment to patient care, with a focus on delivering personalized healthcare services to individuals, particularly seniors and those with disabilities.
13 week contract
Must have at least 12 months of home health experience within the last 3 years
HCHB and OASIS charting experience is required.
Monday-Friday 8 AM- 5 PM weekend requirements every 4th weekend with limited on-call.
$2,000/Wk plus .42 a mile for patient related travel
Duties
- Administer medications and injections as prescribed, ensuring adherence to safety protocols.
- Perform comprehensive assessments of patients' health status, including vital signs and medical history.
- Develop and implement individualized care plans in collaboration with the healthcare team.
- Provide wound care, IV insertion, and phlebotomy as needed.
- Monitor patients’ progress and adjust care plans accordingly.
- Educate patients and their families about health management, medication administration, and lifestyle adjustments.
- Maintain accurate records of patient care activities and report any changes in condition to the healthcare team.
- Foster a supportive environment for patients with disabilities, ensuring their needs are met with dignity and respect.
Requirements
- Current Registered Nurse (RN) license in the state of practice.
- At least 12 months experience in home health care is required.
- HCHB and OASIS charting experience is required.
- Strong background in patient care, particularly for seniors or individuals with disabilities.
- Ability to work independently while managing multiple patient cases effectively.
- Excellent communication skills to interact with patients, families, and healthcare professionals.
