Overview
We are seeking a compassionate and dedicated Hospice Nurse to join our team. In this role, you will provide high-quality nursing care to patients with life-limiting illnesses, ensuring comfort and support for both patients and their families. Your expertise will be vital in delivering holistic care tailored to the unique needs of each patient.
13 week contract- Must have at least 12 months of Home Health and/or 12 months of Hospice within the last 3 years.
HCHB is required
Monday- Friday 2 PM-10 PM
$2,050-$2,150/wk plus .42 a mile for patient related travel

Duties
- Assess and monitor patients' vital signs and overall health status.
- Develop and implement individualized care plans in collaboration with interdisciplinary teams.
- Provide direct nursing care, including medication administration, wound care, and symptom management.
- Educate patients and families about disease processes, treatment options, and end-of-life care.
- Support families emotionally and provide guidance on coping strategies during this challenging time.
- Coordinate with other healthcare professionals to ensure continuity of care.
- Document patient progress and maintain accurate medical records.
- Participate in team meetings to discuss patient care strategies and outcomes.
Qualifications
- Valid nursing license in the state of practice.
- HCHB is required
- At least 12 months in home health or/and hospice within the last 3 years
- Strong ability to assess vital signs and recognize changes in patient conditions.
- Excellent communication skills to effectively interact with patients, families, and healthcare teams.
- Compassionate demeanor with a commitment to providing high-quality end-of-life care.
