Overview
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. This role is pivotal in coordinating patient care, ensuring that patients receive the appropriate services and support throughout their healthcare journey. The ideal candidate will possess a strong background in patient care, utilization management, and have experience in various medical settings, including hospital medicine and geriatrics.
8 week contract-
Must have at least 12 months hospice experience within the last 3 years
HCHB required
$2,000-$2,100/Wk plus .42 a mile for patient related travel
Friday at 5 PM to Monday at 8 AM weekly.
Responsibilities
- Conduct comprehensive assessments of patients' medical records to develop individualized care plans.
- Collaborate with multidisciplinary teams to coordinate patient care and ensure adherence to treatment protocols.
- Monitor patient progress and adjust care plans as necessary, ensuring optimal outcomes.
- Facilitate communication between patients, families, and healthcare providers to promote understanding of treatment options.
- Ensure compliance with HIPAA regulations while managing sensitive patient information.
- Utilize knowledge of hospital medicine and geriatrics to provide specialized support for patients in various settings, including ICU and Level I Trauma Centers.
- Participate in utilization management activities to optimize resource use and improve patient care efficiency.
Requirements
- Current Registered Nurse (RN) license in the state of practice.
- Experience in hospice is required-At least 12 months within the last 3 years
- Hands on use of HCHB
- Excellent communication skills with the ability to work collaboratively within a team environment.
- Strong organizational skills with attention to detail in managing patient cases effectively.
- Commitment to providing high-quality patient care while adhering to ethical standards.
