Overview
We are seeking a compassionate and dedicated Registered Nurse - Home Health to join our team. In this role, you will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. You will collaborate with a multidisciplinary team to develop and implement individualized care plans that address the unique needs of each patient. This position requires strong clinical skills, effective communication, and a commitment to patient education.
13 week contract
Must have at least 12 months of home health experience within the last 3 years
HCHB and OASIS are Required
Monday-Friday 8 AM- 5 PM 40 hours per week
$2,000/Wk plus .42 a mile for patient related travel

Responsibilities
- Conduct comprehensive assessments of patients' health status and needs in their home environment.
- Develop and implement personalized care plans in collaboration with healthcare providers and family members.
- Administer medications and treatments as prescribed, including Botulinum toxin treatment when applicable.
- Monitor patients' progress and adjust care plans as necessary based on clinical findings.
- Provide education and support to patients and their families regarding health management, including primary care practices.
- Document all patient interactions, assessments, and interventions accurately using medical documentation standards.
- Collaborate with interdisciplinary teams to ensure continuity of care, including coordination with teaching hospitals when necessary.
- Assist patients with developmental disabilities by implementing appropriate nursing interventions tailored to their needs.
- Utilize Athenahealth software for effective patient management and documentation.
