Overview
We are seeking a dedicated and compassionate Registered Nurse (RN) to join our Home Health team. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. This role requires a strong commitment to patient-centered care, effective communication skills, and the ability to work independently. As a Home Health RN, you will play a crucial role in helping patients manage their health conditions while promoting recovery and enhancing their quality of life.
13 week contract- Must have at least 12 months of home health experience within the last 3 years.
HCHB and OASIS is Required.
Monday-Friday 40 hours per week. Work every 3rd weekend (day off during the week).
Rotation of on call one weekend to every 3rd weekend. Monthly on call of 2-3 times per month.
$2,000/ plus .42 a mile for patient related travel
Duties
- Conduct comprehensive patient assessments and develop individualized care plans.
- Administer medications and treatments as prescribed, including medication administration and acute pain management.
- Provide health coaching to patients and their families regarding disease management and wellness strategies.
- Perform diagnostic evaluations to monitor patient progress and adjust care plans as necessary.
- Assist with family planning services as needed.
- Educate patients on the use of medical equipment and self-care techniques.
- Collaborate with interdisciplinary teams to ensure coordinated care for patients.
- Maintain accurate documentation of patient care activities in compliance with healthcare regulations.
- Ensure adherence to infection control protocols, including sterile processing when applicable.
Experience
- Valid Registered Nurse (RN) license in the state of practice.
- Previous experience in home health or community-based nursing is preferred.
- Familiarity with specialized skills such as mammography, botulinum toxin treatment, dermal fillers, or operating room (OR) procedures is a plus.
- Strong critical thinking skills and the ability to make sound clinical decisions independently.
- Excellent interpersonal skills for effective communication with patients, families, and healthcare professionals.
