Job Overview
We are seeking a compassionate and dedicated Hospice Nurse to join our team. In this role, you will provide high-quality nursing care to patients in a home or community setting, ensuring comfort and dignity during their end-of-life journey. You will work closely with patients, families, and interdisciplinary teams to deliver personalized care plans that address both physical and emotional needs.
8 week contract
7 days on/7 days off Monday – Thursday 5:00 PM to 8:00 AM and Friday 5:00 PM to Monday 8:00 AM
Must have at least 12 months hospice experience within the last 3 years
Responsibilities
- Conduct comprehensive assessments of patients' health status and needs.
- Develop and implement individualized care plans in collaboration with the healthcare team.
- Administer medications and treatments, including tube feeding and botulinum toxin treatment as required.
- Provide triage support for urgent care situations, ensuring timely interventions.
- Maintain accurate medical records and documentation in compliance with regulatory standards.
- Educate patients and families on disease processes, symptom management, and care strategies.
- Coordinate case management activities, facilitating communication among healthcare providers.
- Support outpatient services as needed, ensuring continuity of care.
Requirements
- Valid nursing license in the state of practice.
- Experience in home community care or hospice settings preferred.
- Proficiency in managing medical records and utilizing electronic health record systems such as Athenahealth.
- Strong skills in triage, case management, and patient education.
- Familiarity with outpatient services is a plus.
- Excellent communication skills and the ability to work collaboratively within a multidisciplinary team.
