Overview
We are seeking a compassionate and dedicated Hospice Nurse to join our team. The ideal candidate will provide high-quality care to patients in a home setting, ensuring comfort and dignity during their end-of-life journey. This role requires a strong commitment to patient-centered care, effective communication with families, and the ability to work collaboratively with a multidisciplinary team.
13 week contract
Must have at least 12 months of Hospice experience within the last 3 years.
HCHB is required
Schedule is every weekend on Friday at 5pm and end on Monday at 8am.
$2,000-$2,100/Wk plus .42 a mile for patient related travel
Responsibilities
- Deliver nursing care to patients in accordance with established care plans and standards.
- Conduct thorough assessments of patients’ physical and emotional needs.
- Administer medications and treatments as prescribed, including catheterization and spinal taps when necessary.
- Provide education and support to patients and their families regarding end-of-life care options and family planning.
- Collaborate with healthcare professionals to develop individualized care plans that address the unique needs of each patient.
- Monitor patient conditions, document changes, and report significant findings to the healthcare team.
- Assist with travel nursing duties as required, ensuring continuity of care across different settings.
- Maintain a clean and safe environment for patients, adhering to all infection control standards.
Skills
- Proficient in triage techniques and the ability to prioritize patient needs effectively.
- Familiarity with home care practices and standards for hospice care.
- Experience in pediatrics or working with individuals with developmental disabilities is preferred.
- Knowledge of botulinum toxin treatment protocols is a plus.
- Strong interpersonal skills for effective communication with patients, families, and team members.
- Ability to work independently while also being an integral part of a healthcare team.
- Flexibility to adapt to changing situations and patient needs.
