As we continue to expand, we are looking for a highly organized, detail-oriented, and compassionate Care Coordinator to join our coordination team.

Job Description:

The Care Coordinator is responsible to coordinate patient services and caregiver staff by scheduling and providing continuous communication to co-workers, management, caregivers, family members/patients, and payors. The Care Coordinator works closely with these individuals and their families to ensure that they receive the services they need in a way that is both effective and efficient.

Responsibilities:

  • Plan, supervise, and coordinate patient services based on authorized hours, ensuring adequate coverage and continuity of care as per assigned caseload and back up caseload.
  • Skill matches the appropriate caregiver to the level of service needed for each patient.
  • On a daily basis, generate and act on case coverage verification reports, specifically “no show”, “call outs”, “open visits”, changes in coverage, exception reports, monitor missed visits, verify Electronic Visit Verification (EVV), unbalanced visits, and address any issues.
  • On weekly basis, review and provide summary of hold, hospitalization, and recent discharges to management.
  • Communicates to caregiver patient’s plan of care and all pertinent rules and regulations.
  • Maintains open and transparent communication with patients, their families, caregivers and contract case managers to ensure a positive care experience, i.e., schedule changes. And documents accordingly in HHAeXchange.
  • Documents and reports to supervisor(s) and clinical team when there is a change in patient status, change in service requirement, change in aide assigned to case, incidents, and complaints.
  • Provide patient advocacy and crisis intervention in collaboration with the Director for Patient Services and Nursing staff.
  • Coordinates, supervises and evaluates caregiver staff. Counsel and discipline caregivers as required.
  • Reviews and approves caregiver requests for personal and vacation days.
  • Must work “on call” and provide “back up” support for other Care Coordinators on a rotational basis.

Qualifications:

  • High school diploma or equivalent.
  • At least one (1) year of home care or health care experience in scheduling.
  • Must possess sensitivity in understanding employees' needs while maintaining a sense of balance to ensure that staffing requirements are met for patients.
  • Demonstrated ability to manage multiple tasks with changing priorities.
  • Superb organization and time management skills.
  • Computer proficiency in Microsoft Word/Excel/Outlook, Electronic Medical Records system, i.e., HHAeXchange.
  • Ability to work well under pressure and comfortable in a fast-paced environment.
  • Excellent written and verbal communication skills.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
  • Detail-oriented with strong organizational and time-management skills.
  • Ability to work independently and as part of a team.
